プラン変更にあたり、以下項目を記載ください。変更手続きには、変更手数料1,100円や、差額（必要な場合のみ）を別途お支払い頂きます。また、プラン変更時は、その時点で保有している繰越回数も無効となりますので、ご注意ください（利用回数を増やすためにプラン変更する場合は除く）。

Please fill out the following form for changing plans. A change fee of 1,100 yen and the difference (if necessary) will be charged separately for the change procedure. Please note that the number of carry-over credits you have at the time of plan change will also become invalid (except when changing plans in order to increase the number of usage).
